JOB SUMMARY:
Provides quality pharmaceutical care defined as being responsible for assuring the provision of safe, efficacious and cost-effective drug therapy for all patients with chronic pain. The clinical pharmacist works in a multidisciplinary practice and collaborates with all team members utilizing a collaborative practice based care team model. The pharmacist provides face-to-face and one-on-one phone based disease management with patients. The pharmacist will utilize integrated and routine communication with all health care professionals and the patients concerning medical care and quality of life with the emphasis placed on the monitoring and documentation of specific pharmacotherapy outcomes. Provides assistance and in-depth drug information to medical personnel; is responsible for Medication Therapy Disease Management(MTDM) activities related to pain management The clinical pharmacist will consult with the patient to identify, resolve and prevent medication related problems. The clinical pharmacist assesses each of the patient?s medications for indication, effectiveness, safety and adherence issues and orders lab tests in the assessment process. The clinical pharmacist will follow up with the patient to titrate/manage/monitor the patient?s medication therapy to ensure positive outcomes. Performs operational/clinical functions or projects as assigned; meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
Performs duties to provide the highest level of patient care and maximize patient satisfaction.
Works under the general direction of the Director, Ambulatory Clinical Pharmacy Programs, Assistant Directors, Ambulatory Clinical Pharmacy Programs and/or Regional Coordinator of Ambulatory Pharmacy Programs.
COMPETENCIES AND SKILLS:
Demonstrates excellent communication and interpersonal skills, especially in working as part of a team. Computer literacy is preferred.
Ability to perform the essential functions of the job as outlined in the position description.
Ability and willingness to train department trainees (eg students, residents, new staff).
Ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients managed by his or her assigned clinic.
Demonstrated knowledge of the principles of MTDM and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
EDUCATION AND/OR EXPERIENCE:
Bachelor of Science in pharmacy or doctor of pharmacy (Pharm D) degree from an accredited college and current licensure or eligibility for licensure by the Pennsylvania State Board of Pharmacy required.
In the absence of a PharmD., a Bachelor of Science in pharmacy with appropriate clinical pharmacy experience will substitute.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
